Important Information About the Forms

Your online thermography intake form is an important part of your exam. It must be filled out thoroughly in order to make proper assessments. Please give adequate information in short points and not long answers.  This allows our doctors to read all of the important information and not lose any information.  We recommend that you maintain your own health history and add to it periodically.

Please have this form filled out 24 hours prior to your exam.  There are protocols that must be followed to ensure accurate results. Please read What to Expect for your appointment. You may also visit FAQ’s.  We want you to have the best experience possible.
